Diabetes Foot Care Program
The Diabetes Foot Care Program is funded by the Mississauga Halton LHIN to help prevent foot complications in patients with diabetes.

The program offers free preventative foot care by Chiropodists and Registered Practical Nurses for patients with diabetes who meet eligibility criteria.

How to Access

Patients MUST be enrolled in a Diabetes Education Program within the Mississauga Halton region. Primary Care Practitioners are NOT able to refer directly into the Foot Care Program.

Active Patients

If you are currently an active patient at a Diabetes Education program in Mississauga Halton contact the Diabetes program directly to request a referral to the Foot Care Program.

NOT being followed by a Diabetes Education Program?

  • Refer the patient to a Diabetes Education Program by filling out a Diabetes services referral form
  • Patients can initiate a Diabetes referral by contacting the Mississauga Halton Central Intake Program 905-338-2571 ext. 4872
  • Once the patient has been seen at the Diabetes program, a clinician will complete an assessment and refer them to the Diabetes Foot Care Program
  • Foot Care Program will contact client directly to schedule appointment
